According to ESRB, this game contains: Blood and Gore, Drug References, Intense Violence

Parents, please keep your children away from this game. It's nothing more than wall to wall violence, mostly of the unbelievably brutal sort. This is one of those times when they really, really mean the M rating.

True Believers will find that this game is packed to the gills with Marvel characters and references, continuing the Marvel videogame tradition of doing a much better job at respecting the Marvel universe than Marvel movies do.

Deaf and hard of hearing gamers will be mostly okay with the game. They lose the advantage of being able to hear nearby gunshots, but other than that, the game is completely playable. All important talk is subtitled, but almost none of the Punisher's eggplant prose is, which removes quite a bit of the character's trademarked style.
